Release of Immunization Records

  • Students must return the completed Release of Immunization Records Form to the Registrars office and show a government issued photo ID before records can be released to the student.
  • A student may call the Office of the Registrar to request release of their immunization records. However, a completed release form must be submitted and ID obtained prior to releasing records.
  • Records can be physically picked up by student, sent via US mail or faxed. Due to the Family Educational Rights and Privacy Act (FERPA), records cannot be released through email.
  • The Office of the Registrar will release the records within three days after a completed immunization records release form has been received.
